What to Bring

what to bring1.) You should bring your identification. Acceptable forms of identification are as follows: Drivers license, State ID, Military ID or Passport. In short, any picture ID that lets us know you are who you say you are.

2.) You should also bring any insurance information you have. If you do not have insurance you don't have to worry about this one. Insurance information would include: ID card (or a claim number for an auto accident). If you are unsure just bring what you have and we will start from there.

3.) The third thing you can bring is any person that might want to come with you. Whether that person is a family member or friend that is concerned for you or even interested in chiropractic care. An additional person is always helpful to ask questions, listen and make you more comfortable with your experience. It is not a requirement, but can be helpful.

Please allow yourself the appropriate time to receive the care needed.
